Clinical Care

Complex care needs that involve health professionals such as RGNs, Community Nurses, Physiotherapists, can often be provided at home.  This is what we refer to as Clinical Care and usually involves 24/7 support from our dedicated and highly trained team.

We can provide specialist Nursing care and support in a number of ways, covering the following specialisms:

  • PEG / NG (specialist nutrition & hydration methods)

  • Diabetes

  • Epilepsy

  • Administering medication via special techniques

  • Cough assist and respiratory management

Working in Partnership

The Local CCG have information on their website on how to apply for an assessment from them with help towards your care. Any assessment is free, whether by the Local Authority, NHS, or a Care Provider

The four stages of how to seek assessment and to see if you are eligible for funding are:

1. Contact Leeds Adults & Health on 0113 222 4401

2. Agree a plan of care based on your needs – this will either be to be supported at home, or it might be more suitable for you to receive residential/nursing care

3. Choose the most appropriate funding options available to you – this can include a Direct Payment (DP), Personal Health Budget (PHB), or to have a Provider paid directly by the CCG / LA

4. Finally, you will be able to choose a Provider from a number of resources. A popular one is the Leeds Directory, found here

Although Providers in the Directory are vetted, it is always good practice to check the most recent CQC report; check they are registered with the United Kingdom Home Care Association (UKHCA); all staff have had an Enhanced DBS check; and there are checks in place to train and monitor their staff
